The tag or 'Erkennungsmarke' shows signs of wear. It belonged to a soldier who was drafted in the Sanitäts Ersatz Abteilung 4. This abteilung was formed in October 1939 in Leipzig from Sanitäts Ersatz Kompanie 4. It trained and supplied soldiers for different units. In April 1943 it became Sanitäts Ersatz und Ausbildungs Abteilung 4.
The 'O' on both parts of the tag stands for the soldier's Bloodtype.
