The tag or 'Erkennungsmarke' shows overall signs of wear. It belonged to a soldier who served/was drafted in Radfahr Ersatz Schwadron 213. If we understand it correctly this unit was part of Aufklärungs Ersatz Abteilung 8 and probably trained and supplied soldiers for Aufklärungs Abteilung 213.
The 'O' on both parts of the tag stands for the soldier's bloodtype.
